Please
set your printer to print only "page 1 of 1" to
save paper.
Your
e-mail address MUST be included. Please make sure it is
CORRECT.
If
you would like for me to upload the photos to your
homepage, or to an online photo album,
please supply me with all the fields required. I suggest
you make a temporary password for these before sending
your photos to me. Then once your photos are scanned in,
change your password again.
Note
that you will receive TWO effects for 50 cents. Any
additional effects are 15 cents each.
Please number your photos
"Photo # 1" through "Photo # 6" this
way I know which effects to create for each photo.
Please
PLEASE make sure that you include a SASE (self addressed stamped envelope) if
you wish your photos to be returned to you. That means
that YOU write your return address on an envelope, put a
stamp on it, and send it in the same envelope with your
photos.
Please
double-check all the information to make sure it is
correct so there will be no confusion and will save time.
When
you are ready to send your order form, you will need to E-mail me for the
address to which to send the order form.
